No bug.
"SCAN" only scans the sets which had problems in a previous "NEW SCAN", so of course the old check settings are restored (exception: fix options).
"NEW SCAN" always scans all sets and then remembers the used settings for future scans.
So with an update for your fav datfile, you first do a NEW SCAN, and from then onwards you only need 'SCAN's.
Now to the sets/roms difference...
Set = a "game", a collection of roms. Look at the datfile, you see the single sets...
E.g. "Pacman" which got let's say 10 roms (1.bin, 2.bin, ....just example names)
So if you check for sets + name, you check if the pacman folder (or pacman.zip/.7z/.rar) is correctly named.
If you check for roms + name, you check if the files inside the pacman set (1.bin, 2.bin,....) are correctly named.
Simply keep all check options enabled....
